Eastern US storms kill 9, cause wide power outages

WASHINGTON (AP) ? Violent storms swept across the eastern U.S., killing at least nine people and knocking out power to millions of people on a day that temperatures across the region are expected to reach triple-digits.

The Mid-Atlantic region had already been experiencing 100-degree temperatures before Friday evening's violent storms. More than 3 million are without power ? and without air conditioning ? as crews work to clear downed tree limbs and restore electricity.

The storms were blamed for the deaths of six people in Virginia; two in New Jersey; and another in Maryland. In suburban Washington, residents were told to call non-emergency phone numbers or go to fire and police stations if they needed help because even 911 emergency call centers were without electricity.

Power outages were reported from Indiana to New Jersey, with the bulk of the service interruptions concentrated in the Mid-Atlantic region. Earlier Friday, the nation's capital reached 104 degrees ? topping a record of 101 set in 1934.

On Saturday, temperatures were expected to reach 100 degrees again ? and another round of storms also was possible. The National Weather Service warned the heat index could reach 110 degrees.

The heat left people such as the elderly vulnerable. In Charleston, W.Va., firefighters helped several residents of an apartment building, some using wheelchairs or walkers, move to a shelter. Fire Capt. Chris Campbell said the evacuation was voluntary and was the only one since the storm hit. But he expected more.

In addition to the heat, officials say cell phone coverage is spotty. Many residents were asked to conserve water because sewage stations had been without power for a time. And authorities cautioned people to drive carefully because tree limbs littered roads and hundreds of traffic signals were out.

No power also meant no way to charge cellphones and laptops, and no Internet access in many areas.

Matthew Pelow, 39, was supervising a 10-man crew spreading 275-degree asphalt in Washington. They kept bottles of water on hand and were working quickly to finish before the heat got any worse. They also arrived in the cooler early morning hours.

"We got here just as quick as we could," Pelow said.

Kim Molisee, meanwhile, sat frustrated in her car outside the normally bustling but now darkened All-Star Express convenience store in Reedsville, W.Va., just down the road from her home. Officials said about 500,000 people were without power in that state.

"I'm almost out of gas, and I can't run around too much trying to find a store that's open where I can get gas and ice," she said.

Molisee and her 13- and 15-year-old sons were at the Walmart in Kingwood when the storm hit and the power failed.

"Me and the boys made a run for it to the car," she said. They made it home safely and suffered no serious damage, "but even after the storm, you could hear the trees falling."

Molisee will be able to cook on her gas-powered stove and even shower because she has a gas water heater. But staying cool is another matter.

"I think I'm gonna get my boys and go to the river," she said.

More than 20 elderly residents at an apartment home in Indianapolis were displaced when the facility lost power due to a downed tree. Most were bused to a Red Cross facility to spend the night, and others who depend on oxygen assistance were given other accommodations, the fire department said.

Those who could afford it flocked to hotels to escape their hot, powerless homes. Others planned to spend the day at places like shopping malls in an effort to stay cool.

Jose Amaya, 41, of Germantown, Md., was also without power on Saturday. He said his wife and two daughters planned to the mall to stay cool and he joked that the outage was going to cost him because they would be shopping. His wife, who works for a hotel chain, also planned to get the family a room to stay.

Robert Clements, 28, said he showered by flashlight on Friday night after power went out at his home in Fairfax, Va. The apartment complex where he lives told his fianc? that power wouldn't be back on for at least two days, and she booked a hotel on Saturday.

Clements' fiance, 27-year-old Ann Marie Tropiano, said she tried to go to the pool, but it was closed because there was no electricity so the pumps weren't working. She figured the electricity would eventually come back on, but she awoke to find her thermostat reading 81 degrees and slowly climbing. Closing the blinds and curtains didn't help.

"It feels like an oven," she said. "It is hot."

Rare Roman find made in Scotland

A rare Roman artefact and a weight set that could be evidence of illegal trade between the Highlands and Europe have been among finds made in Scotland.

A copper alloy cast in the shape of an eagle head uncovered in Selkirk is the first Roman relic of its kind to have been found north of the border.

The weights dating from the 1700s were unearthed at Fortrose.

The finds, along with others made in Perth and Kinross and Fife, are mentioned in a new report.

The seventh annual Scottish Treasure Trove report catalogues ancient and historic items found between April 2011 and March this year.

The Queen's and Lord Treasurers Remembrancer (QLTR) and the Scottish Archaeological Finds Allocation Panel (SAFAP) dealt with the discoveries.

Under Scots law, the Crown can claim any archaeological objects found in Scotland.

Finders have no ownership rights and must report any objects to the Treasure Trove Unit.

QLTR Catherine Dyer said the new publication confirmed another "magnificent year" of finds.

She added: "Some outstanding and very significant finds have been reported, preserved and displayed in museum collections around Scotland.

"Once again I would like to praise the tireless work of the Scottish Archaeological Finds Allocation Panel, the National Museums of Scotland, the Treasure Trove Unit and the QLTR office.

"Thanks should also be given to the hundreds of members of the public who have played an important part by reporting their finds and in doing so have assisted in preserving the history of Scotland for all to enjoy."

The copper eagle head found in the Borders would have been a feature on a Roman wagon.

Only a small number are known to have been uncovered in Britain and none previously in Scotland.

The complete weight set from Fortrose, in the Highlands, was made in Nuremberg and calibrated for Emden, a city in Germany.

Use of the measurements would not have been legal in Scotland's burghs, according to the Treasure Trove report.

However, this type of set were commonly found in markets on the fringes and perhaps suggest that an "unruly free enterprise" existed between Scotland and European trade networks, the report added.

William Bradley: The 'Slow Boring of Hard Boards' in an Era of Limits

"We are entering an era of limits. In place of a manifest economic destiny, we face a sober reassessment of new economic realities, and we all have to get used to it.

We can't just ignore the demands of social and economic justice or the fragile environment on which we all depend. But in meeting our responsibility, we are now forced to make difficult choices.

Freeways, child care, schools, income assistance, pensions, health programs, prisons, environmental protection -- all must compete with one another and be subject to the careful scrutiny of the common purpose we all serve.

The "hard board" of California politics, boring though it may be at times, is getting bored. This month has brought some more progress on the chronic state budget crisis, the beginning of some results for reform efforts, and, as the state Republican Party continues its devolution, telling early returns on the appeal of independents.

Governor Jerry Brown's budget compromise with Democratic legislative leaders has been adopted.

Brown insisted that California's welfare system become more like that advocated by his old campaign rival, former President Bill Clinton.

He moved Democratic legislative leaders further along in wringing more long-term savings from the chronically unbalanced state budget. The deal requires federal work requirements being met after two years, with only limited exemptions, reduces child care funding, and stops cost of living increases starting next year. It will also move nearly 900,000 children to Medi-Cal from the more expensive Healthy Families program.

These are not exactly accomplishments for which it is appropriate to sing songs of praise, but do provide some needed savings for the budget and political bona fides for Democrats needing to pass their November revenue initiative. With the budget and its trailer bills handed off to Brown, there is also the need for major some action on public pension reform, and the start of construction of high-speed rail is a decision that looms.

Brown is also moving successfully to cut employment costs by 5 percent. After balking at his initial plan for a shorter work week, employee bargaining units are coming around to furloughs. Which, ironically, they hated when Governor Arnold Schwarzenegger imposed them.

Meanwhile, the big water bond passed at the tail end of Schwarzenegger's administration, the first big water legislation passed in decades, is still set for the November ballot, a problematic situation in this very challenging fiscal environment if it is not moved and/or substantially altered, especially with no sense of a drought.

Brown last week dropped his current move to block conservative attempts to use the California Environmental Quality Act to block and/or delay construction of the project. Too many environmental groups whose support is necessary to move the bullet train construction authorization -- funds are already at hand for the first phase from the federal government and approved bonds -- through the legislature were balking at any change in the law, fearing that a one change for a green project could lead to other changes for other projects. Notice that I said he dropped his current move.

The state's new open primary system, adopted after the passage of a 2010 initiative championed by Schwarzenegger, was coupled this year with the new districts drawn by the Citizens Redistricting Commission, also championed by Schwarzenegger in a 2008 initiative.

Former Democratic legislator and state finance director Steve Peace, a longtime champion of open primaries, worked with a bipartisan crew of strategists to aid in the initiative's passage. Democratic strategist David Townsend, a Brown adviser, former Schwarzenegger media consultant Don Sipple, who also advised Brown in his 2010 bid, Schwarzenegger re-election campaign manager and McCain for President director Steve Schmidt, and one-time chief of staff to Republican Governor George Deukmejian Steve Merksamer joined with longtime Schwarzenegger consultant Adam Mendelsohn and others to ensure the measure's passage in 2010.

Today Townsend calls the early outcome a success. It's not a panacea, he says, but "it's already making a real impact."

Like another key Brown backer, former Democratic legislator and state parks director Rusty Areias, Townsend saw advances for more moderate Democrats. There were also fewer Republican candidates signing on to the mindlessly reflexive no-tax pledge pushed for years by Washington Beltway lobbyist Grover Norquist.

A report by the Public Policy Institute of California (PPIC), surveying the district outcomes so far, concluded that "The new districts have disrupted established incumbents, while the top two primary has altered traditional patterns of competition. The reforms led to closer outcomes."

The open primary and the end of the legislature's gerrymandering of districts is now joined by another reform, albeit on the margins. That's the term limits revision, which passed easily, 61-39, with the undecided breaking heavily in its favor.

Legislators will see the number of years they can serve cut from 14 to 12, but all of them can now be spent in the same house.

The term limits revision could restore greater functionality to the legislature by ending the crazy musical chairs that term limits have engendered, as well as ending the preposterous situation in which new members are not infrequently chairs of committees.

The tobacco tax initiative on the June primary ballot finally went down to defeat in the final count last week, as I expected. But it was closer than expected, losing by only 30,000 votes.

The measure, which would have tacked another dollar a pack on cigarettes to generate cancer research funds, which is, let's say, a not exactly unknown field of endeavor, saw its goo-goo appeal run down by a $50 million advertising campaign by the obviously self-interested tobacco companies.

Yet, even though the initiative was contending with a conservative-skewing low-turnout primary electorate with a policy idea I thought was played out, so much I so I didn't support it -- cancer research is not exactly an unsupported field, and this measure seemed like another way to try to remove an admittedly unhealthy pleasure from low-income people -- it nearly won anyway.

Last week saw another very prominent California Republican, the highest ranking yet, leave the party to become an independent. That's former California Secretary of State Bruce McPherson, appointed to the office by then Governor Arnold Schwarzenegger. McPherson was a longtime state legislator prior to the Arnold nod. He is running for Santa Cruz County Supervisor, in a race in which he was already the frontrunner.

McPherson is a moderate conservative, which means that in today's far right anti-Enlightenment California Republican Party he's viewed as an untrustworthy "liberal."

While his registration switch should work -- he's won many races in the Santa Cruz area -- changing horses in mid-stream didn't work out so well for state Assemblyman Nathan Fletcher, who finished at 24 percent, nearly doubling his standing before he switched, but behind Republican Carl De Maio at 32 percent and Democrat Bob Filner at 30 percent.

I wondered when he made the switch -- just 17 days after pushing hard for the Republican endorsement, which he lost to far right San Diego City Councilman Carl De Maio -- if his move would look opportunistic. It actually broke both ways. Becoming an independent propelled him from a distant third-place tie with San Diego County District Attorney Bonnie Dumanis into major contention, and a brief tie for the lead. But, though Brown praised him for being willing to seek compromise, the move opened Fletcher up to withering fire from both parties, neither of which wanted to see his gambit succeed.

De Maio's camp was so concerned that, in addition to heavy attacks on Fletcher and his move, they also ran advertising to boost the underfunded Democrat in the race, Congressman Bob Filner, repeatedly citing him as "an Obama Democrat." All the better to boost his Democratic vote and lift him past Fletcher -- well positioned for a run-off that did not come -- into the November run-off.

What this means is that Republicans who seek higher office and don't want to kowtow to a hard right party line are going to have to make the independent move sooner rather than later in order to build credibility.

As Fletcher senior adviser Matt David, who was Jon Huntsman's presidential campaign and, as Schwarzenegger's communication director, helped pass the open primary measure, noted, Fletcher had little time to build a power base independent from his longstanding Republican Party ties.

Had he made the move a year or two earlier, however, and never sought the Republican endorsement, Fletcher might well have seen a very different outcome. As it was, he was well-positioned for the run-off against De Maio, which of course never came.

Brown, who didn't actually endorse Fletcher, got a nice boost last week when he attended a fundraiser with Vice President Joe Biden in Sacramento at the venerable 123-year-old Sutter Club.

Biden called Brown "the smartest guy in American politics."

"Nothing has changed," Biden said of Brown, who was governor before from 1975 to 1983. Biden said he met Brown in San Francisco in the 1970s. He said, "He was the smartest guy in American politics then. He's still the smartest guy in American politics."

"He speaks his mind," Biden said, with no little sense of irony. "I like guys like that."

The sliding Republicans don't have anyone to run for governor against Brown, who will be only a year-and-a-half into his term on July 3, and no Democrat is going to take on a Brown, especially this Brown, in a primary. But his governorship gets even less fun if his plans for the year don't work out.

Go Commando Half Marathon to feature Free Health and Fitness Expo

Go Commando Half Marathon and 5kClarksville, TN ? The Go Commando Half Marathon & 5k powered by CDE Lightband will feature a free Health and Fitness Expo as part of the race weekend. The Go Commando Health Expo presented by The Runners Hub will be Friday, October 19th from 10:00am?8:00pm.

The event is open to the public as well as race participants.

The Health Expo will be held at the new Wilma Rudolph Event Center at Liberty Park. Race participants will have the chance to pick up their event packets and meet health and fitness professionals and vendors.

Over 45 vendors will be on hand to offer advice, training tips, provide samples and more. The Runners Hub, presenting sponsor of the Health Expo, will have a micro-store on-site offering all the running essentials.

(AP) ? Doubts over the ability of European leaders to cobble together measures to deal with the debt crisis kept markets on edge Thursday.

Stocks fell and the euro edged lower as expectations for the European Union summit that starts later in Brussels diminished in the wake of comments from German Chancellor Angela Merkel that jointly-issued eurobonds would not resolve Europe's debt crisis on their own.

Many in the markets as well as a number of Europe's leaders, including those from France, Italy and Spain, think eurobonds are a key ingredient in the resolution of the crisis as they would help lower indebted countries' borrowing costs.

But Merkel is reluctant to expose Germany to new potential costs and is concerned that such bonds would ease the pressure on countries like Greece and Spain to reform their economies.

"In the past, there's been a lot of optimism leading into these summits, sending stocks and other risky assets higher," said Craig Erlam, market analyst at Alpari. "With so much experience now of these summits producing nothing, a wait and see approach is now being taken. "

In Europe, Germany's DAX was 1.6 percent lower at 6,127 while the CAC-40 in France fell 1.1 percent to 3,029. The FTSE 100 index of leading British shares was 0.9 percent lower at 5,472.

Much of the FTSE's fall was due to the performance of its banks, notably Barclays PLC, which dropped around 5 percent a day after it was slapped with fines totaling $453 million for the manipulation of key interest rates.

Wall Street was also poised to open lower, with both Dow futures and the broader S&P 500 futures down 0.7 percent.

The euro was also being dogged by lack of optimism over the summit and was trading 0.4 percent lower at $1.2430.

Figures showing that German unemployment fell only modestly to 6.6 percent in June ? a month that usually sees big demand for seasonal jobs ? added to the prevailing sense of unease.

Earlier, a few Asian markets posted gains as they tracked gains in the previous session in Europe and the U.S. following positive housing and manufacturing reports out of the U.S. on Wednesday.

Japan's Nikkei 225 rose 1.7 percent to 8,874.11. South Korea's Kospi rose marginally to 1,819.18 while Hong Kong's Hang Seng gave up earlier gains to fall 0.8 percent to 19,025.27.

Mainland Chinese shares lost ground, with the benchmark Shanghai Composite Index down 1 percent to 2,195.84, the lowest closing in almost five months. The Shenzhen Composite Index lost 1 percent to 909.28.

Oil prices fell below the $80 a barrel mark as sentiment in the markets turned sour. Benchmark oil for August delivery dipped 36 cents at $79.82 a barrel in electronic trading on the New York Mercantile Exchange.

The reality of today?s funding environment for digital health entrepreneurs is that it?s traditional tech investors who have the lion?s share of the money, while most long-time healthcare investors are on the ropes, contending with fleeing LPs and at least the?perception of disappointing returns.

While it?s great news that some tech funds seem interested in dipping their toes into the healthcare space, it?s concerning that the investors with the most resources are not necessarily the ones who understand healthcare the best.

Tech investors, in general, are not always comfortable with physicians, and seem much more at home with engineers and developers.? These investors also tend to gravitate to businesses selling directly to consumers rather than dealing with the sordid complexities of our current healthcare system.

Many tech investors are also ??understandably ? drawn to the power of data, and the possibility of analytics, a sensible affinity but one that at times can translate into an excessively reductive view of medicine that fails to capture the maddening but very real ambiguity of medical science, and especially of clinical practice.

While almost everyone contemplating the problems of the current healthcare system acknowledges that often stubborn, intransigent doctors can be part of the problem, it?s arresting how many technologists don?t also view doctors as a key element of the solution; instead, there seems to be a common, techno-utopian ?vision in which medicine has been profoundly disrupted, and the role of physicians largely replaced by computers ? see?here (a particularly vivid example), as well as?here.

In the words of digital health investor and doctor Bijan Salehizadeh, ?IT rooted VCs don?t know how to relate to or speak the language of physicians. They bow at the altar of revolutionary disruption. That works in non-regulated consumer markets. ?Not in highly regulated hierarchical fragmented spaces like HCIT.?

The more technology investors I speak with, the more I worry there?s something elemental about medicine that many don?t seem to understand ? and worse, they don?t always know what they don?t know.

Doctors have a unique perspective

The experience of being a doctor provides unique insight into the meaning and significance of health.

For starters, as a physician, you develop a profound, overwhelming appreciation of how serious health is, and you experience and help patients and their families in dealing with every phase of illness.? When I listen to a young entrepreneur extol his or her new approach to?saving large files orsharing music as an example of changing the world, I?appreciate and love their enthusiasm and passion ? how can you not?? At the same time, I also find myself reflecting upon other talks I?ve heard ? Judah Folkman discussing his discovery of angiogenesis, or Bruce Walker discussing advances in HIV therapy ? and I think: now this ??this is changing the world.

Second, for all the emphasis placed in medicine on arriving at a diagnosis and suggesting a treatment, the truth is that medicine is just so much less precise than anyone could possibly imagine. Diseases, diagnoses, best practices: all are so much grayer and less-well demarcated than most appreciate, and patients are far more complex. For all the advances in molecular diagnostics and high-tech imaging, medicine remains far more scientism than science.

Third, once you?ve had the experience of caring for patients, had the privilege of developing relationships with patients over time, it?s impossible not to be moved by the power, complexity, and importance of the doctor/patient connection. Twenty-something Palo Alto engineers may struggle to understand why anyone would waste time with human care providers, but I suspect the many patients with serious concerns or chronic conditions who are fortunate enough to receive regular care would likely offer a very different perspective on the value of this unique relationship.

At the same time, while physicians may genuinely aspire to deliver the best care possible, they demonstrably struggle achieving this; many rely on vague recollections rather than current data, and are notoriously reluctant to discard bad habits (such as not washing hands), and embracing new recommendations (such as utilizing a basic checklist), despite the demonstrated evidence for both.

The real opportunity in medicine, then, is not to replace physicians, but to enable them, and enable patients. This will entail recognizing the foundational value of the close, longitudinal relationship between doctor and patient, and then building off of this by creating tools for both patients and physicians that will enhance this therapeutic connection. To develop such tools, a company will need to combine deep healthcare knowledge with technological sophistication, and create solutions that deliver not just clicks, but durable improvements in patient health.

Areas of opportunity and challenge

One category of opportunity here is turning mHealth into mMedicine (to borrow two terms from Dr. David Albert, founder of AliveCor). Right now, there?s an overwhelming number of consumer-focused wellness apps and fitness gadgets, products like the Eatery and Fitbit which may be pleasant to use, and could potentially support health. The real opportunity is robustifying these sorts of measurement technologies and approaches, so that they not only provide casual entertainment, but could also be used to drive and monitor real medical benefit ? see here and here for a discussion on the importance of these sorts of measurements in medicine.

A second opportunity is behavior modification ? a staggering amount of illness results from the many poor choices each of us make every day, habits that are notoriously difficult to change. The degree of user engagement engendered by many gaming and social apps is remarkable, and could be harnessed to drive measurement improvements in health. Early versions of this have been used for employee wellness programs, but I suspect there are real opportunities for platforms that can maintain patient interest over time (a huge problem right now), and which can nudge the patient towards improved behaviors and better health.

A third category: care delivery/health system improvement. It?s difficult to identify an individual aspect of the current care system ? from hospital admissions to managing illness at home ? that doesn?t appear to be a historical relic, and which seems thoughtfully designed with the patient in mind. There are so many opportunities to rethink aspects of this, and ask how could this process be better constructed ? ideally incorporating key aspects of design thinking. Traditionally, healthcare innovation has focused on new drugs and devices, but there?s an increasing recognition of the need to improve systems as well ? a key focus of leading-edge initiatives such as Dr. Arnold Milstein?s Clinical Excellence Research Center at Stanford, aimed at improving healthcare services.

A fourth category: Chunking big data. While the idea of chasing big data in health is certainly not new, figuring out how to actually acquire the relevant data remains non-trivial, and turning this information into actionable insight remains an abiding challenge ? with the key word being actionable, i.e. usable by physicians, or patients, in as frictionless ? and ideally delightful ? a fashion as possible. Success will require far more than robust analytics. Combining clinical and payor data, meanwhile could be used to support value-based healthcare, highlighting the best way to allocate limited resources. This information could used to improve the quality and efficiency of care, and also could be provided to other financial stakeholders (which will likely increasingly include patients themselves) to support informed decision making, a la Castlight.

A key challenge that all entrepreneurs face, and which seems especially pronounced in the healthcare space, is finding not only an important problem to solve, but a way to get paid for solving it. The perverse incentives in the current healthcare system mean that in many cases, physicians and hospitals are actually rewarded for not being better ? they often make money on the extra charges that suboptimal care can generate.

This may explain better than anything else why tech investors like consumer plays ? investors understand them, and know how to assess the value, how to monitor, and when to pivot. They?ve also made a lot of money on at least some of these companies, and understand the business model.

In contrast ? and unfortunately ? the business case for improving care can be surprisingly difficult to make; it?s likely that entrepreneurs may need to carefully identify a specific situation (a concierge practice? A private oncology clinic?) where their proposed improvements can demonstrate the most compelling impact. It?s also why some forward-thinking healthcare entrepreneurs such as Avado?s Dave Chase have focused so much of their efforts on the need for incentives better aligned with patients? interests.

Partnerships for the Future

While there?s not going to be a magic formula for digital health companies, a leadership team combining healthcare and technology experiences seems a common pattern.
